Obviously not the biggest rivalry in world sports, but around here the biggest sporting rivalry is NSW vs QLD in State of origin.
It's a series of 3 all star games played in the middle of the season, but the players play harder and take it more seriously than the finals. The star players represent the state where they grew up. It has this magic history of extremely close contests and epic clutch fairy tale moments. The players literally are willing to die on the field to win, the intensity it is played at has legitimately been measured by science to be far greater than how they normally play. These 3 games a year are just everything.
